Welcome! The link below will take you to the surveys for the study. Please review the following instructions prior to entering the surveys.

  1. You will next be asked to enter your unique Study ID. The research team should have given this to you when you completed the informed consent. If you did not receive an ID or you have lost your ID,  please contact the research team at 301-295-1951.

  2. The survey asks questions about mental health symptoms. If you are not comfortable answering a question, you may skip it, but please try to answer all of the questions.

  3. At the end of the mental health symptoms survey please click 'Submit'.

  4. After you click 'Submit' you will be shown a short demographic survey. At the end of the demographic survey, please click 'Submit'.

  5. If it any time while taking the survey you experience distress, please contact the research team at 301-295-1951. We are available during normal business hours (0800-1700 EST M-F). If we miss your call we will do our best to get back to you within 24 hours. If your need is more urgent, please contact the Military Crisis Line at 1-800-273-8255 and pressing 1. You can also reach the Military Crisis Line by texting 838255. The Military Crisis Line is available 24/7 and is confidential. There is more information available on their website, including a chat feature. If you are experiencing a mental health emergency, dial 9-1-1 or your location's appropriate number for emergency services.
